What is Proteus?

Proteus mirabilis is an enterobacterium of the normal gut flora, highly mobile, that readily climbs the urinary tract. It is found at any age, somewhat more often in people with a urinary catheter, in older men and in children with a urinary tract anomaly — but also in people with no particular factor.

Its specialty: it alkalinises urine

Proteus produces an enzyme, urease, which turns the urea in urine into ammonia. Two very concrete consequences. First a strong, ammonia-like odour that patients often describe before the result even arrives. Second, alkaline urine: this high pH favours a particular type of kidney stone (struvite), in which the bacteria can lodge. That is the key point about this germ: a colonised stone works like a reservoir, and the infection can come back after each treatment as long as the cause is not addressed.

Why Proteus infections recur

If your urine cultures keep finding Proteus, tell your doctor: they can look for a stone or another favouring cause, sometimes with imaging (an ultrasound for instance). Treating the infection without addressing a possible stone is like emptying a bucket that keeps refilling — dealing with the cause is what breaks the cycle.
